    Athletic Applications, Brochures & More Information


    Each student must have a complete Athletic & Physical Registration Form on file BEFORE trying out for a sport. Your medical physical form is good for 24 consecutive months. You’ll also need to read and sign the Concussion and Sudden Cardiac Arrest Awareness Form after reading the Concussion Information Sheet and Sudden Cardiac Arrest Information Sheet. Please fill out the appropriate sport form below and return all completed paperwork to Mr. Simcha in the gym.
