WAMS COVID-19 Vaccine Clinic

The pediatric Pfizer COVID-19 vaccine is now approved for children 5-11 years old. SPS is excited to partner with Public Health – Seattle & King County and Othello Station Pharmacy to offer COVID-19 vaccination clinics for students five and older right here at Washington Middle School.
School: Washington Middle School
Date: Tuesday, November 23rd (First Dose)
Time: 9:30am – 11:30am
No pre-registration. School-day clinics are only for students enrolled in that school where the clinic is held.
The Second Dose clinic will be on Tuesday, December 14th, 9:30 am-11:30 am
If your student is not able to make it to this clinic, there are many other ways to get your child vaccinated, including your regular doctor’s office or clinic, your local pharmacy, as well as community vaccination sites, and community clinics. Requirements for Parents/Caregivers
A parent/guardian must give consent for a student to be vaccinated. Consent forms are below. Please get in touch with the school nurse if you would like a paper copy sent home.
Students may attend school-day clinics without a parent/guardian if the parent/caregiver has signed a consent form in advance. A parent/caregiver may also give verbal consent to a school nurse. Please contact your school nurse to do this. Paper consent forms will be sent home with students.
