The pediatric Pfizer COVID-19 vaccine is now approved for children 5-11 years old. SPS is excited to partner with Public Health – Seattle & King County and Othello Station Pharmacy to offer COVID-19 vaccination clinics for students five and older right here at Washington Middle School.

Requirements for Parents/Caregivers

A parent/guardian must give consent for a student to be vaccinated.  Consent forms are below.  Please get in touch with the school nurse if you would like a paper copy sent home.

Students may attend school-day clinics without a parent/guardian if the parent/caregiver has signed a consent form in advance. A parent/caregiver may also give verbal consent to a school nurse. Please contact your school nurse to do this. Paper consent forms will be sent home with students.
