Athletic Applications, Brochures & More Information

Registration

Each student must have a complete Athletic & Physical Registration Form on file BEFORE trying out for a sport. Your medical physical form is good for 24 consecutive months. You’ll also need to read and sign the last page of the Concussion and Sudden Cardiac Arrest Awareness Form. Please fill out the appropriate sport form below and return all completed paperwork to Mr. Sherlock in the gym.
